Recommendations for the management of patients with aneurysmal subarachnoid hemorrhage

After SAH, primary and secondary complications are frequent and often require neurosurgical interventions to avoid secondary brain damage. The authors of the present paper have summarized the available data about the treatment modalities often used for patients with SAH. Image-guided transcranial Doppler sonography for monitoring of vasospasm after subarachnoid hemorrhage.

Transcranial Doppler ultrasound (TCD) is a standard method for bedside vasospasm monitoring after subarachnoid hemorrhage (SAH). Image guidance has previously been shown to reduce intra- and interobserver variability of this method. The aim of the present study was to compare image-guided and conventional TCD in vasospasm monitoring after SAH.418 TCD exams of 24 consecutive SAH patients registered in a database were evaluated. Surgically verified variations in the A1 segment of the anterior cerebral artery

✓ Two patients with rare variations of the A1 segment of the anterior cerebral artery are presented. One patient had a perforation of the optic tract by an abnormal course of the A1 segment, and the other harbored an aneurysm of the A1 segment running below the optic nerve. The authors present a summary of A1 segment variations described in the literature.

First-ever stroke incidence and 30-day case fatality in the Sicilian Aeolian archipelago, Italy

Background and Purpose— Not many data on stroke epidemiology come from studies on islands. This is the first report on a Mediterranean archipelago population. Methods— Using recommended criteria, from July 1, 1999, to June 30, 2002, information was collected on first-ever stroke and 30-day case fatality in Aeolian island residents (13 431). Results— The overall crude incidence rate was 154 of 100 000 (95% CI, 118 to 197; 128 in men and 180 in women) or 180, 154, and 87, if adjusted to the Italian, European, and world populations, respectively. The role of endothelium-related factors in the pathogenesis of cerebral vasospasm after subarachnoid hemorrhage (SAH) has gained interest since the discovery of endothelin-1 (ET-1). We have examined, before and after SAH, the responsiveness of the cerebrovascular bed of the goat to ET-1, the sources of Ca2+ in ET-1-induced responses, and the ability of the Ca2+ entry blocker nicardipine to counteract them. Low-Frequency Therapeutic Ultrasound with Varied Duty Cycle: Effects on the Ischemic Brain and the Inner Ear

Sonothrombolysis is a promising modality for acute stroke treatment. In vitro data suggest a duty cycle dependence of sonothrombolytic efficacy of low-frequency applications. The aim of our study was to examine its impact on safety issues in a rat model of middle cerebral artery occlusion. Rats were exposed to transcranial 60-kHz ultrasound with varied duty cycles. To determine effects on the inner ear, the acoustic threshold was determined in additional healthy animals (acoustic evoked potentials).
